To apply for our school Nursery please print out and complete the below form and return it to the school office using the dropbox below or email awcontact@acklamwhin.co.uk and attach the mentioned documents to process your application.

The following documents must be seen and photocopied by the office:

•  Your child’s Birth Certificate (original)
•  Proof of address (child benefit letter/household bill / council tax, gas etc
